6.     Candidates will submit applications in duplicate to the DGAFMS, Ministry of Defence, NEW DELHI, on the form as may be prescribed for this purpose. Theprescribed form may be obtained from the same office in response to anadvertisement appearing in the press calling for such applications. Applications will be accompanied by the following documents :-

(a)     Certificate of Age and Nationality. A certificate of age and nationality under the rules prescribed by t he Government of India and signed by the authority permitted to issue such certificates under the existing rules of the Government of India.

(b)    Character Certificate. A recommendation and certificate of moral character from two responsible persons, not being members of the candidate's own family, to the effect that he is of regular and steady habits and is likely, if appointed, to prove in every respect creditable to the AMC. One of these persons should preferably be an officer of the Armed Forces.

(c)      Evidence of Registration of Medical Qualification. As required vide para 3(b) above.
